  • What you need:

    • Cover sheet

    • Copy of your most recent LES (MyPay)

    • DA31

    • How to fill out instructions (or example copy)

    • Individual medical readiness (IMR)

      • Go to MedPros, scroll down and click IMR Record

    • Itinerary OR vehicle inspection

OCONUS

  • What you need:

    • Cover sheet

    • DA 31

    • Copy of your most recent LES (MyPay)

    • Individual medical readiness (IMR)

      • Go to MedPros, scroll down and click IMR Record

    • Flight itinerary

    • Passport

    • ATL1

    • CTIP

    • SERE 100.2

    • ISOPREP- within 6 months of travel dates

    • Completed at bldg. 17

    • APACS: status approved

TDY
